Position Overview: The Home Doctor is seeking a compassionate and dedicated Nurse Practitioner to join its team. As a Nurse Practitioner specializing in primary care, you will play a pivotal role in delivering comprehensive healthcare services to our diverse patient population across various living settings. Responsibilities: Provide direct patient care, including conducting comprehensive assessments, diagnosing health conditions, prescribing medications, and developing personalized treatment plans. Collaborate closely with the interdisciplinary team to ensure coordinated and holistic care delivery. Conduct regular patient visits within their living environments, emphasizing continuity of care and personalized attention. Educate patients and their families on health maintenance, disease prevention, and treatment options. Maintain accurate and updated patient medical records using electronic health records (EHR) systems. Adhere to clinical protocols, evidence-based practices, and regulatory standards while delivering care. Demonstrate sensitivity, empathy, and cultural competence in addressing patients’ diverse needs and backgrounds. Qualifications: Master’s degree in Nursing (MSN) from an accredited Nurse Practitioner program. Current and unrestricted Nurse Practitioner license in [state/region]. Certification as a Nurse Practitioner specializing in adult-gerontology or family practice. Prior experience in primary care, preferably in a mobile or home-based healthcare setting preferred but not necessary. Strong clinical assessment and diagnostic skills. Excellent communication, interpersonal, and teamwork abilities. Ability to work independently and manage a flexible schedule.
