This role ensures continuity of patient care, regulatory compliance, efficient staffing operations, and consistent performance across all departments.As a senior leadership position, the Director works cross-functionally to align clinical, operational, and administrative teams while driving performance, accountability, and continuous improvement throughout the organization.Key ResponsibilitiesProvide strategic and operational leadership for care coordination, staffing, and administrative teamsEnsure uninterrupted patient coverage and effective utilization of caregivers across all authorized casesOversee scheduling practices, including live-in, split, and high-acuity cases, ensuring compliance with labor laws and agency policiesPartner with Human Resources, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention teams to resolve escalations and staffing challengesMonitor and enforce documentation standards, including scheduling changes, case notes, and coordination actionsAddress caregiver and patient concerns related to scheduling, communication, or service delivery and implement corrective actions as neededTrack and analyze key operational metrics such as unstaffed hours, overtime usage, response times, and service complaintsReport performance trends and improvement initiatives to executive leadershipWork EnvironmentThis role is based in an office setting with frequent interaction with staff and patients. The position may involve managing time-sensitive issues and operating in a fast-paced, high-accountability environment.QualificationsRequired:Minimum of 2+ years of experience in home care coordination or operationsAt least 5+ years of leadership or management experience within home health or home careStrong knowledge of New York State home care regulations and labor requirementsDemonstrated ability to manage high-volume, fast-paced teamsExcellent communication, problem-solving, and decision-making skillsPreferred:Experience within a Licensed Home Care Services Agency (LHCSA)Familiarity with HHA Exchange or similar home care management systemsExperience overseeing multi-region or high-census operationsWhy This Role MattersThe Director of Coordination plays a critical role in maintaining patient continuity of care, reducing service disruptions, managing labor costs, and improving both caregiver and patient satisfaction within a growing organization.Equal Opportunity Employer (EOE)
