Position Overview: Lincoln Street, Inc. is seeking a Temporary Traveling Registered Nurse licensed in the State of Vermont to provide clinical support across our residential and community programs throughout Vermont. This role is ideal for a travel nurse, contract nurse, or community health nurse who enjoys working in diverse settings and supporting individuals with developmental disabilities.
The Traveling Nurse will ensure individuals receive quality healthcare through assessments, direct support at physician or specialist appointments, medication oversight, staff training, and coordination with healthcare providers.
Key Responsibilities:
· Provide nursing support across multiple program sites
· Perform health assessments, wellness checks, and ongoing monitoring
· Coordinate medication services with prescribers
· Maintain accurate medical documentation and health records
· Coordinate care with physicians, specialists, and healthcare providers
· Provide medical training and support to direct care staff and Home Providers
· Assist with care plans, health protocols, special care procedures and regulatory compliance
· Respond to medical concerns and urgent health situations
Qualifications:
Current RN license in Vermont (required)
Minimum 1–2 years of nursing experience (community health, behavioral health, or residential care preferred)
Ability to travel between program sites and appointments (valid driver’s license required)
Strong organizational and communication skills
Ability to work independently and collaboratively with multidisciplinary teams
Experience supporting individuals with developmental disabilities is a plus.

Pay: $34.40 - $41.43 per hour
