Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 04/27/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel RN - Case Manager Location: Lebanon, New Hampshire Pay: $2672/week Schedule: Day Shift: 8:00 AM - 4:40 PM (Mon-Fri) 40 hours/week No weekends or on-call Duration: 13 weeks Start Date: 04/27/2026 Job Details: Manage daily caseload of inpatient units with diverse care needs Perform initial patient assessments and develop care plans Collaborate with patients, families, and interdisciplinary teams Coordinate discharge planning (home health, rehab, DME, etc.) Communicate patient progress and post-discharge needs Document and place orders using EPIC EMR Participate in interdisciplinary rounds Requirements: Active RN license BSN required Certified Care Manager required Minimum 3 years inpatient clinical experience At least 1 year of care management experience Strong communication and coordination skills - Medical & Health Insurance Benefits from First Day for All Clinical and Patient-Facing Employees. - 401K Plan. -"Your Way is Paid" - we pay for ALL employment requirements, onboarding, physicals, titers, etc. - $300 - $1000 referral bonus.
