Occupational Therapist Location: Haxtun, CO Agency: Mindlance Health Pay: $2,326 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 5/18/2026 About the Position ID: 77007820 Shift: 8hr Days 8x Description: HWL Main Street Clinic-NANC-0017 OT needed at Haxtun! 8 or 13 week contract, flexible for 4-10's or 5-8's. Department is open from 7a-6pm. Guarantee 36 hours! inpatient/swing, outpatient, extended care unit. Min 1 yrs OT experience required , CO license required ( can be pending at time of submission). EMR is Athena. We have 5 inpatient/swing beds, 20 extended care beds and an OP clinic. HWL prescreen with manager final interview and offer Minimum Qualifications: Bachelors or masters degree in occupational therapy from an American Occupational Therapy Association (AOTA) or the Accreditation Council for Occupational Therapy Education (AOTE) certified school. Current license as an Occupational Therapist Registered in the State of Colorado. Basic Life Support (BLS).
