What you'll do in this role: * Hospice patient identification, follow-up, and management in the hospital * Manage Hospice referrals for assigned area, which may come from several sources including but not limited to: Hospitals, Discharge Planners, Physicians * Work closely with the insurance department * Review charts to determine eligibility for program(s) * Determine Hospice level of care needs for each referral * Call/ conduct visits with patients/caregivers to explain and discuss Hospice services * Process appropriate documents including physician orders/certification * Communicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient We are looking for compassionate Hospice Admissions Registered Nurse with: * RN license in the state you work * Associate degree or higher from an accredited School of Nursing * Two years of RN experience, hospice experience preferred * Current CPR and BCLS certification * Current driver's license We are an equal opportunity employer and value diversity at our company.
