Our mission is to improve healthcare outcomes for our patients, providers, and partners, guided by our core values.Role OverviewThe Nurse Practitioner will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating with the Center Leadership Team to meet specific center goals and priorities, as well as working alongside the Medical Director and/or Attending Physician to provide tailored medical care for patients.The Nurse Practitioner will deliver primary and urgent care services to residents, working in partnership with other medical providers to fulfill our mission of enhancing lives through high-quality healthcare and compassionate service.QualificationsGraduation from an accredited School of Nursing program.Current licensure as a registered nurse.Certification as a Nurse Practitioner from an accredited program, with a Master's Degree in Nursing, specializing in Older Adult and/or Family Nurse Practitioner (FNP).A minimum of two years of experience working with adult or geriatric populations is preferred.APRN with an unrestricted professional license and DEA registration for each state of practice.Working ConditionsThis position includes a comprehensive 90-day training program along with ongoing support.
