Location: Field BasedSchedule: Full-time Monday–Friday + occasional weekends (as needed)Compensation (Productivity Points): $58/PPWho We AreRole: Physical Therapist Assistant (PTA)Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care.The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes — in partnership with a supervising PT.We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time.What You Will DoDeliver skilled PTA visits in the home under the supervising PT’s plan of care.Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.Coach patients and caregivers with practical home programs that actually get done.Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.Why AdaptiveOperations that back you up: You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.Clear productivity model with upside: You are paid per productivity point/visit (see PTA comp model) with clear expectations.Schedule flexibility: We build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible.Cutting-edge tech built for clinicians: Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes.Mission-driven work: We are here to expand access to high-quality home health care, and we want clinicians who take pride in raising the standard.RequirementsMust-havesActive Texas PTA license (in good standing)Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearlyStrong communication and documentationReliable follow-through on treatment plans, visit cadence, and patient goalsCurrent driver’s license and comfort with daily travel across assigned territoryAble to carry/transport basic therapy equipment as neededCompetency in core home health PTA scopeSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device educationStrengthening, functional mobility, and home safety assessment/modification educationPatient/caregiver coaching with practical home programsNice-to-havesPrior home health experienceHomecare Homebase (HCHB) experience (or similar home health EMR)Deeper clinical experience in high-signal ortho/neuro populationsStrong organization and time managementCompensation Range: $75K - $100K
