Home Health Physical Therapist Assistant

Job description: We are a small, therapist-led home care/field based physical therapy practice in Queens, NY, focused on providing high-quality, personalized care to Medicare Part B patients in the comfort of their own homes. Our mission is to deliver effective, compassionate, and convenient care to adults and seniors who want to maintain their independence and mobility. We are also hiring PTA’s who have a desire to work with pediatric patients in the home health setting. We are looking to bring on a reliable, communicative, and motivated Physical Therapist Assistant (PTA) to help us meet growing patient demand. You’ll be working under the supervision of a licensed Doctor of Physical Therapy and will have the opportunity to build your caseload gradually with strong administrative support. Responsibilities: Provide in-home physical therapy services to Medicare Part B patients Carry out an established plan of care developed by the supervising physical therapist Deliver care focused on mobility improvement, pain reduction, fall prevention, and post- hospital or post-home health recovery Complete timely and accurate clinical documentation using a HIPAA-compliant system Communicate regularly with the supervising PT regarding patient status, progression, or any concerns Coordinate scheduling directly with patients or support staff as needed Maintain a high standard of professionalism, reliability, and respect for patients’ homes and privacy Ensure a safe and clean working environment by adhering to infection control protocols Who We’re Looking For: Required: Active New York State PTA license Strong clinical skills and confidence working independently in the field Excellent communication and documentation habits Reliable transportation to travel within Queens Must be comfortable working as a 1099 independent contractor Preferred: 1+ years of experience in outpatient, home health, or geriatric physical therapy The ideal candidate will possess a strong understanding of medical terminology, physiology, and anatomy to effectively support patient recovery Compensation and Details: Pay: $50/hour (per visit) Type: 1099 independent contractor (no benefits) Flexible schedule — you control your availability Caseload will start part-time (5–10 visits/week) with the opportunity to grow to 25–30+ visits/week All patients have Medicare Part B with physician-approved plans of care How to Apply: Please submit your resume outlining your experience, availability, and why you’re a good fit for this role.
