Registered Nurse Job Description Job Overview: Able Family Services, a HHSC contracted provider for the HCS and TXHML Medicaid waiver programs, is seeking a dedicated Registered Nurse to join our team. As a Registered Nurse at Able Family Services, you will play a crucial role in providing high-quality care to individuals with Intellectual and Developmental Delays. Duties: - Participate in the admission process of new clients selecting Able Family Services as their agency provider. - Attend SPT, IP, and IPC meetings to contribute valuable insights and recommendations. - Delegate tasks to staff as necessary, including ISS staff, while supervising LVNs effectively. - Ensure that medical records are up-to-date and that a current list of medications by physicians is maintained before completing a CNA. - Develop and implement plans to meet the medical and healthcare needs of clients, ensuring appropriate follow-up care. - Provide clear instructions to clients, HHCC staff, and support staff regarding medical conditions, allergies, diets, etc., and offer ongoing consultation on healthcare matters. - Coordinate annual nursing assessments with clients and HHCC care staff. - Maintain accurate documentation on individual health statuses for various support personnel. - Make referrals for necessary medical services and ensure follow-up care is pursued diligently. Skills: - Bachelor's Degree preferred. - Medicaid Waiver (HCS) experience preferred. - Current Nursing License in the state of employment. - Minimum 12 months of experience as an RN. - Ability to establish supportive relationships with individuals receiving supports. - Proficiency in taking instructions and constructive criticism. - Possession of a valid Driver's License. Working Conditions: - May be On-Call 24 hours a day for medical emergencies in collaboration with other management staff. - Remote work opportunities available with flexible scheduling options. - Preferred experience with basic computer skills and utilization of a printer/scanner.
