Job Opportunity: Physical TherapistOur organization is in search of a committed and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. In this position, you will deliver evidence-based rehabilitative care to adult and elderly patients in various clinical environments, assisting them in regaining function, enhancing mobility, alleviating pain, and achieving their maximum level of independence and quality of life.Key ResponsibilitiesConduct thorough patient evaluations, including assessments of musculoskeletal, neuromuscular, and functional capabilities to identify tailored treatment requirements.Design and execute patient-focused care plans that integrate therapeutic exercises, manual therapy, gait and balance training, functional mobility training, and pain management strategies, following physician directives and established clinical guidelines.Regularly evaluate patient progress, adjust treatment plans as necessary, and provide clear education to patients, families, and caregivers on home exercise routines, safety measures, equipment usage, and methods to sustain functional improvements.Work collaboratively with a multidisciplinary team, including physicians, nurses, occupational and speech therapists, social workers, and other healthcare professionals, to coordinate care, facilitate discharge planning, and ensure optimal clinical outcomes.Accurately document all assessments, goals, interventions, and patient responses in the electronic health record, complying with regulatory, payer, and organizational standards.Working ConditionsIn line with our organization's inclusive culture, which emphasizes dignity, respect, and professional growth, you will help foster a supportive and nurturing atmosphere for patients, families, and colleagues.
