Position Overview: The Community Engagement Specialist will serve as a vital bridge between the healthcare organization, patients, families, and community resources. This role is dedicated to enhancing community engagement, advocating for patient needs, and ensuring that individuals have access to comprehensive healthcare services and resources. The Community Engagement Specialist will foster strong relationships, promote health initiatives, and support the overall mission of Prime Providers with improving community health outcomes. Key Responsibilities: Community Engagement: Develop and maintain strong relationships with local community organizations, schools, facilities, hospitals, Regional Centers, and support groups. Represent the healthcare organization at community events, health fairs, meetings, and forums to promote awareness and services offered. Patient and Family Support: Provide direct support to patients and families, assisting them in navigating the healthcare system and accessing necessary services. Offer guidance on available resources, including educational programs, therapeutic services, and financial assistance. Coordination of Services: Collaborate with healthcare providers, social workers, case managers, service coordinators, and other professionals to ensure a coordinated approach to patient care. Facilitate communication between patients, families, and healthcare teams to address concerns, plan of treatments, and monitor progress. Communication: Facilitate clear and consistent communication between the organization and the community. Gather and relay community feedback to the appropriate departments to influence program development and improvement. Develop and distribute informational materials to keep the community informed of the organization's activities. Advocacy: Advocate for the needs of patients within the healthcare system and the broader community. Work with community leaders to improve access to services and support for the community. Education and Outreach: Develop and deliver educational programs and materials for patients, families, healthcare providers, and community members. Organize workshops, in-services, and support groups to educate and empower individuals regarding healthcare resources and available services. Resource Management: Maintain up-to-date knowledge of local, state, and national resources available for patients and their families. Create and manage a resource directory for patients, families, and healthcare providers. Growth Management: (Expansion and Referral Oversight) Responsible for driving business growth by identifying opportunities and managing referral processes. This includes fostering relationships to encourage referrals and ensuring effective strategies are in place to support organizational development. Additional Responsibilities: Employees may be assigned various tasks outside their primary duties as required. Flexibility in handling diverse responsibilities that support the overall goals of the organization is expected. Qualifications: Associate’s degree in the field of Social Work, Public Health, Healthcare Administration, or a related field. Bachelor’s degree preferred or subsequent experience may be considered. Minimum of 3 years of experience in community outreach, healthcare, or a related setting. Strong knowledge of healthcare systems and community resources. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Experience in advocacy, community outreach, or patient support services. Proficiency in Microsoft Office Suite and database management. Personal Attributes: Compassionate and empathetic, with a genuine commitment to supporting patients and their families.
