Life Care Private Duty & Community Wellness Nurse (RN)Location: Rochester, New HampshireHours: Part Time 24 hours, all the way up to full time (by pick up hours in Home Care or Hospice)Organization: Cornerstone VNACornerstone VNA is seeking a compassionate and community-focused Registered Nurse to join our team as a Life Care Private Duty & Community Wellness Nurse. This role supports our mission to promote the optimum level of well-being, independence, and dignity of those living in the community by providing trusted, compassionate, and expert health care.This is a dynamic position combining in-home clinical care, staff collaboration, and community wellness programming, with a strong emphasis on coordinating and providing foot care clinics, which are a required component of the role.Position SummaryThe Life Care Private Duty & Community Wellness Nurse provides clinical assessments, develops and manages care plans, supervises and collaborates with staff, and coordinates community wellness initiatives. The nurse also plans and conducts wellness clinics in community settings such as senior centers and local employers, including regularly scheduled foot care clinics.Key ResponsibilitiesClinical Assessments and Care Planning* Conduct home visits to assess client needs, including physical assessments and health history.* Develop, implement, and evaluate individualized plans of care.* Perform recertification visits for CFI and VA patients as indicated.* Respond to significant health changes or concerns with timely home visits.* Participate in co-visits with Life Care staff to support quality care and adherence to care plans.Community Wellness and Clinics (Required Focus)* Coordinate and conduct foot care clinics as a regular and essential responsibility of the role.* Plan and implement vaccination clinics, cholesterol screenings, and other wellness programs.* Provide health education and coaching to community members and staff.* Coordinate vaccine ordering and related logistics.Staff Collaboration and Support* Collaborate with the Community and Life Care Director and other clinical leaders.* Make referrals to appropriate programs and services.* Assist with training and educational programs for care staff.QualificationsRequired* Current New Hampshire Registered Nurse (RN) license.* Minimum of 2 years of RN experience.* Comfort working both independently in the community and collaboratively with a team.* Willingness and ability to lead and perform foot care clinics as a routine part of the position.Work Environment* Community-based role with travel throughout the service area.* Combination of home visits, clinic settings, and community locations.* Frequent standing, walking, and occasional lifting consistent with nursing duties.What We Offer* Competitive salary* Excellent benefits package including:* Medical, Dental, and Vision* 401(k) Retirement Plan* Life and Disability Insurance* Flexible Spending Account* Paid Time Off* Mileage Reimbursement* Education Reimbursement* Pet Insurance* Identity Theft Protection* Will Preparation Services* Supportive team environment and ongoing professional developmentIf you are passionate about preventive care, community outreach, and helping people remain healthy and independent at home-and are comfortable leading foot care clinics as a core responsibility-we encourage you to apply.
