If you're looking to advance your career in a supportive environment that values excellence, autonomy, and teamwork, we'd love to meet you.Position: Registered Nurse (RN) - Home HealthPay Range: $50.00-$65.00/hrStatus: Full-TimeCoverage Area: Coachella ValleyWhat You'll DoThe Registered Nurse (RN) plans, organizes, and coordinates home health services while delivering high-quality, patient-centered nursing care. The RN uses strong clinical skills and community-based nursing experience to support individuals and families in their homes, ensuring safe, effective, and compassionate care.Develop and initiate the patient's individualized plan of care and lead ongoing, interdisciplinary assessments in collaboration with the patient, their representative, and caregiversImplement appropriate preventative, therapeutic, and rehabilitative nursing interventions as ordered by the physician and outlined in the plan of careProvide thorough patient and caregiver education, including disease management, medication management, safety, and home-based care techniquesIdentify discharge planning needs early in the care process and ensure all necessary steps and education are completed prior to dischargeCoordinate delivery of medical equipment, supplies, and additional services to ensure patients have what they need for safe, effective care at homeSupervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and Home Health Aides (HHAs), ensuring care is delivered in accordance with regulatory and organizational standardsRequirementsValid CA Registered Nurse License2+ years of nursing experience, public health nursing or home health experience preferredMaintains a current CPR certificationMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.Why Join Bella Terra Home Health & Hospice of the Desert?Bella Terra is part of the Cornerstone Group, a nationwide network of 75+ home health and hospice agencies.
