TCOOMMI Case Manager

StarCare Specialty Health System seeks outstanding applicants passionate about supporting individuals with mental illness and substance use disorders who are criminal justice involved. The TCOOMMI (Texas Commission on Offenders with Medical and Mental Impairments) Case Manager,provides pre-release screening and referral to aftercare treatment services for special needs offenders. TCOOMMI Case Manager Responsibilities: Work is performed under minimal supervision of the Coordinator of TCOOMMI Services with broad latitude for the use of initiative and independent judgment. Provides rehabilitative and service coordination services following guidelines set by TCOOMMI as evidenced in the current contract. Services are provided in-vivo and are related to an approved individual plan of service. Services Include: Providing hands-on, in-vivo skills training Symptom management Medication monitoring Crisis Intervention and peer support Addressing substance abuse, criminal justice, housing, money management and employment issues Communicating pertinent clinical information Providing face to face contact with customers and their support systems Inpatient services as needed Other responsibilities include: Evaluating, monitoring, linking, and coordinating consumers on assigned caseload Ensuring consumers are aware of resources necessary to reach personal outcomes Ensuring Center resources are utilized appropriately Completing and submitting required reports Establishing and maintaining good rapport with probation and parole officers. Clinical supervision is provided by the division's LPHA. Schedule:This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department. Qualifications: Graduation from an accredited four (4) year college or university with major course work in a related field, (2) years full-time experience in a related field. Must meet clearance requirements for working in the State prison system. Knowledge, Skills & Abilities Knowledge of human service delivery systems. Ability to communicate effectively, both orally and in writing. Ability to organize, prioritize a variety of assignments and manage time effectively. Ability to maintain an effective working relationship with other employees and the public. Knowledge of appropriate rules, regulations, and policies and procedures. Prefer a solid knowledge of psychiatric terminology, DSM IV, and psychopharmacology. Bilingual capabilities preferred. Knowledge and skill in operating standard office equipment (phone, copier, fax, etc) and personal computer with Microsoft Office products is required. Additional Requirements: Must pass a pre-hire drug screen and criminal background check. Must have a Texas driver's license and be insurable under the Agency's insurance.
