Community Navigator
The Arc of Greater Boone County is hiring a Community Navigator. The Community Navigator is a full-time position working Monday-Friday 8am-4pm. The Community Navigator hourly wage is $18.00 per hour. The Community Navigator will complete the following tasks.
Identify community resources and activities pertaining to the interest and skill level of each person supported.
Maintain relationships with a variety of community resources to support the creation of opportunities for persons served.
Facilitate the participation in integrated community activities (those used by the general public), events, and organizations in locations of the person’s choice in ways similar to other persons of the same age and interest.
Foster connections between persons supported and persons without disabilities to increase relationships and social capital.
Maintain electronic records including progress of goals identified on each person’s person-centered plan, incidents, challenges, etc.
Accompany persons to activities, if needed, and facilitate involvement.
Communicate progress, successes, and challenges to guardians, care providers, and other team members as needed.
Ensure safety of persons on caseload.
Provide support to maintain or enhance personal independence and perceived competency.
Provide supports as needed including personal care, medication assistance, meal supervision, etc.
Support The Arc GBC’s Mission, Vision and Core Values while empowering each person to direct her or her own life choices.
Develop schedules, lessons and activities for persons supported.
Provide needed information for personal profiles that support career and life planning, ISP’s, and Team Meetings.
Work with team to develop appropriate programming.
Plan daily schedules with input from persons served.
Assess community skills of each person and help them in building skills.
Requirements:
Must be 18 years old
Must have valid driver's license
Must have reliable transportation and valid car insurance
Must be able to pass background check and drug screen
Experience:
Experience working with people with disabilities preferred.
Strong communication skills required
Familiarity with community resources/activities and the ability to connect people with disabilities with those resources/activities
High level of professionalism
