If you're energized by helping others, value one-on-one care, and want work that truly matters, this is a strong opportunity.Key responsibilitiesProvide personal care support including bathing, grooming, dressing, oral hygiene, and toiletingAssist with mobility, transfers, ambulation, and range-of-motion exercises per care planPrepare meals and assist with feeding as neededSupport medication self-administration through reminders (no independent med administration)Monitor and report patient condition and changes to the supervising nurseTake and record basic vital signs when trained and assignedMaintain a clean, safe, and orderly client environmentAssist with linens and light client laundry as appropriateUse safety equipment properly (rails, supports, stools, etc.)Document care provided according to company and regulatory standardsTravel reliably to patient homes and assigned visits across your service areaComplete required in-service training, including annual dementia educationWhat success looks likePatients feel safe, respected, and well cared forOn-time arrival and dependable attendance for assigned visitsAccurate, timely care documentationPositive feedback from patients and familiesCompletion of required certifications and training on scheduleStrong communication with supervising nurse and care teamMust-have requirementsAge 18 or olderActive HHA or CNA certification or willingness to obtain certificationWillingness to complete competency and skills assessmentsAbility to follow written care instructions and document servicesCompassion, patience, and professionalism with vulnerable populationsAbility to work independently in home settingsReliable transportation and ability to travel to client homesAbility to lift up to 25 poundsWillingness to complete TB testing, background screening, and onboarding requirementsNice-to-have6+ months of caregiving or direct patient care experienceHome health or home care experienceCPR and Adult First Aid certificationExperience supporting seniors or medically complex clientsExperience working with dementia or memory care patientsWork environment & scheduleOne-on-one care in patient homes and occasional facility settingsFlexible scheduling options may be available (full-time, part-time, PRN depending on market)Assignments may include homes with pets or smokersTravel within your preferred coverage areaIdeal candidate profileAre licensed -- or motivated to become licensedEnjoy helping others and building trust with clientsAre calm, respectful, and safety-focusedCan handle sensitive or challenging situations professionallyAre dependable and punctualCommunicate clearly with patients, families, and nursesWant meaningful, people-centered workIf you're ready to make a daily difference in someone's life -- we'd love to meet you.Are you searching for a purpose?
