What You’ll Do Provide comprehensive, individualized nursing care in home and community settings. Conduct thorough patient assessments, including vital signs, physical exams, and ongoing evaluations. Manage complex care needs Deliver specialized care to a wide range of patients, including pediatric, adult, geriatric (memory care and dementia), and individuals with developmental or behavioral health needs. Educate patients and families with patience and compassion—covering medications, nutrition, safety, and disease management. Coordinate care with physicians, case managers, facilities, and interdisciplinary teams to ensure smooth transitions and continuity of care. Maintain accurate, timely documentation in EMR/EHR systems while upholding HIPAA and infection-control standards. Respond calmly and effectively to urgent or changing patient needs. What We’re Looking For A Registered Nurse with strong clinical skills and a heart for home health. Experience across a variety of care settings (ICU, ER, medical-surgical, post-acute, or specialty care preferred). Comfort working with diverse populations—from pediatrics to geriatrics. Ability to support caregiver training and continuing education efforts, including skills check-offs, clinical guidance, and ongoing competency development. Proficiency with EMR/EHR systems (Epic, Cerner, Athenahealth, eClinicalWorks, or similar). Strong critical-thinking skills and the ability to manage complex, individualized care plans. Excellent communication skills and a compassionate, patient-first mindset. A team player who values collaboration, professionalism, and empathy.
