Segue Health-- Faith Based Medical PracticeSegue Health, a faith based medical practice, is seeking a compassionate Care Coordinator to join our team serving patients in the Plano, TX area coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.QualificationsMust have experience in the medical fieldMust have a knowledge of Microsoft Excel and the ability to learn EMR/EHR softwareShould have transportation to and from local hospitalMust have a heart to serve othersDaily tasks include: Working with referring facility to generate list of patients eligible for TCM servicesVisiting referrals while in the facility to explain the TCM program and determine if there are any anticipated needs post dischargeDaily logging of referrals and dischargesCreating charts in EMR and uploading face sheet, H&P, and DC SummaryPost discharge calls to patients within 24 hoursScheduling of patients preferably prior to discharge for in home visitsOur most successful candidates have experience in social work, nursing, and/or home health.Attention to detail and organizational skills are MOST important.Must provide proof of vaccinations or meet vaccination requirements per hospital standardsJob Type: Full-timeSalary: $45,000.00 - $55,000.00 per yearCompetitive Benefits ProvidedSchedule: 8 hour shiftMonday to FridayAbout Segue HealthSegue Health is a mission-focused organization dedicated to delivering exceptional, Christ-centered care to patients in their homes and communities.
