Certified Health Advisor

A company is looking for a Health Advisor to provide care coordination and support for at-risk patients. Key Responsibilities Conduct post-discharge follow-up calls with patients to assess needs and reinforce care plans Review and reconcile discharge medications and assist in coordinating follow-up appointments Identify high-risk patients and connect them with appropriate care management resources Required Qualifications Certified Emergency Medical Technician (EMT) or Medical Assistant (MA) Preferred 3-5 years of experience in an outpatient healthcare setting Experience with electronic medical records (EMR) systems Proficient in basic technology platforms and able to navigate multiple systems
