Coverage Needed: June 13, 2023 - Ongoing Tuesday 7am to Tuesday 7am Hospital Call Coverage 1:5 Call Rotation Clinical Information: EMR: EPIC Patient Volume 5-10 Per Day Procedures: colonoscopies and EGD Required: Board Certified Active IN License Must have license in hand at time of submission, or proof that the license application has been submitted CV must be in Month / Year format Any gaps greater than 30 days will require an explanation on the CV Active DEA DEA must remain in state to maintain privileges.? Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active Current Flu Vaccine Must be fully vaccinated for Covid 19 Exemptions only allowed for religious or medical reasons, once approved by the facility.
