Job Description
Job SummaryWe are looking for experienced candidates to assist and support adult individuals with developmental and physical disabilities, with the necessary life skills to be as independent as possible in their community. The Direct Support Professional will provide daily living skills, supportive program services, and assistance to individuals in a day support center and community-based settings. ResponsibilitiesAssist clients with Activities of Daily Living (ADLs) including personal hygiene, grooming, and dressing.Provide hands-on care and support, where needed.Monitor client health by observing vital signs and documenting changes in condition.Implement care plans tailored to individual needs while adhering to HIPAA regulations.Assist with meal prep, setup and cleaning.Maintain a safe, clean, and germ-free environment through regular cleaning tasks.Engage clients in social activities and enhance social interactions to promote mental stimulation and well-being.Administer medications as prescribed and ensure compliance with medication schedules.Assist with mobility needs, including the use of wheelchair, walker, rollater, etc.SkillsAssist and support individuals with daily living, such as assistance with eating, dressing, restroom use (when needed), etc.Ensure the health, safety, and welfare of individuals by complying with Medicaid, Office of Licensing, Home and Community Based Settings Rights Policy, DBHDS, and OSHA regulations; as well as CBHS policies and procedures.Administer prescribed medication and assist with self-administration of medication, per the company's policies and procedures.Assist with housekeeping tasks, cleaning and disinfecting, organizing, etc.Capability to multi-task and adapt to change in an ever-changing environment.Support and assist each individual with activities, such as natural support, participation in leisure activities, and the development of appropriate social behaviors.Participate in the development of obtainable goals and outcomes for the individual's person-centered plan, documentation of those services, and activities scheduled for each individual.Be able to report any evidence of abuse, neglect, and other health and safety issues immediately to Management. Complete incident reports per company policies and procedures to Management for review.Develop and maintain an open line of communication for relevant information about individuals with the appropriate staff members and Management.Interaction with individuals, staff, and Management should adhere to the company's Standard of Conduct.Have a great personality, dedicated compassion and an overall team player.Other responsibilities and duties as assigned.
