If you are a proactive, organized person and like building relationships with people, this role may be a great match for you.Role MissionAs the first direct point of contact for Bunny Home Care, the Care Coordinator serves as the bridge between our other teams. You are responsible for taking the "warm hand-off," representing our brand values in the field, and ensuring every new member and caregiver is signed, trained, and successfully integrated into our care ecosystem.As a Care Coordinator, you will be responsible for:Directly manage a high-volume caseload of 100+ members, performing regular field visits to monitor care quality and ensure long-term satisfaction and retention.Main Liaison to our members and caregivers, representing Bunny Home Care as first and direct point of contact.Visiting patients in their homes as needed to coordinate and manage care.Facilitate comprehensive onboarding, compliance and hands-on training for new caregivers to align their skills with company standards and member-specific needs.Maintain compliance with caregiver clock-in and clock-out criteria.Ensure the needs of clients and caregiver(s) are met accordingly ensuring client satisfaction.Handle last-minute call-outs and arrange coverage.Conduct in-home Care Consultations with potential members and families to present care solutions, secure formal signatures, and initiate the enrollment process.Represent the company in the community and in members' homes, maintaining a professional presence that builds trust and reinforces the Bunny Home Care brand.Plan and execute a schedule that ensures each client has at least one quality assurance every 60 - 90 days.Qualifications3+ Years' experience in Home Care or Service Coordination or Health Care Industry.A vehicle to get to clients in a determined range.The ability to work independently in a field position, with a self-starting, upbeat, positive, go-getter attitude.Excellent organizational skills, problem-solving skills, and customer service.Team player attitude with a warm, friendly, compassionate demeanor with elderly clients.Demonstrated capability of maintaining strict confidentiality.Excellent communication, public relations, and follow-up skills.Bilingual speakers are preferred.??
