Physical Therapy Assistant (PTA)The Physical Therapist Assistant (PTA) at Continental Springs in South Sioux City, Nebraska will work under the direction and supervision of a licensed Physical Therapist to provide rehabilitative services to residents in our skilled nursing facility. The PTA will assist in implementing treatment plans aimed at improving mobility, strength, balance, and overall functional independence while ensuring compliance with regulatory and clinical standards.
Key Responsibilities:
Implement treatment plans developed by the Physical Therapist to improve residents' functional mobility and quality of life
Provide therapeutic exercises, gait training, transfer training, balance activities, and modalities as directed
Monitor resident response to treatment and report progress or changes to the supervising Physical Therapist
Accurately document treatment sessions, progress notes, and billing information in accordance with facility policies and regulatory requirements
Assist with patient evaluations as directed by the Physical Therapist
Educate residents, families, and caregivers on exercise programs, safety, and mobility techniques
Ensure compliance with Medicare Part A/B guidelines, PDPM requirements, and state/federal regulations
Maintain a safe and clean therapy environment
Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and social services
Participate in care plan meetings and discharge planning as appropriate
Qualifications:
Graduate of an accredited Physical Therapist Assistant program
Current PTA license in the state of practice
Previous experience in a skilled nursing facility or geriatric setting preferred
Knowledge of rehabilitation techniques, equipment, and safety protocols
Familiarity with Medicare guidelines and documentation requirements preferred
Strong communication and interpersonal skills
Ability to work effectively as part of a team
Work Environment:
Skilled nursing / long-term care setting
Frequent standing, walking, bending, and assisting residents with mobility
May require lifting or supporting residents with appropriate equipment
