Registered Nurse (RN)-Case Manager

The case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:Completing intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to dischargeDeveloping a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family's needsAssisting providers in determining patient's level of care in accordance with InterQual criteriaActively seeking out collaboration with providers to improve overall quality and efficiency of care .Requirements:Graduation from an accredited school of nursing and a Bachelor's of Science of Nursing preferred; applicants holding a valid RN license and currently matriculated into a BSN program may be considered .Prior experience in utilization review, discharge planning and/or case management preferred.3-5 years of recent clinical experience required.VT Nursing License required by date of employment.Case Management Certification required within 2 years of employmentArea of Interest: Nursing;Pay Range: $36.00-$46.00;Work Status: 8:00a-4:00p;Employment Type: Full Time;Job ID: 6141Dartmouth Health offers a total compensation package that includes a comprehensive selection of benefits.
