We are dedicated to making a difference in the lives of employees, offering the opportunity to be associated with caring teammates, and creating positive contributions in each community we serve.What you must have:Graduate of an accredited diploma, associate or baccalaureate school of nursingCurrent state registered nurse licenseCurrent driver's license, reliable transportation, and provide ongoing valid and current auto liability insuranceOne to three years of experience as an RN in a clinical care setting, hospice or home health preferred.Full time employee benefits:Competitive payMedical, dental & vision insurancePaid time offPaid holidays401k with up to 4% employer matchingTuition reimbursementCompany car for qualifying individualsMileage reimbursementWhat you will do:Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.Review clinical referral information and responds to requests and inquiries as appropriateCollaborate with patient care manager, attending physician and hospice physician regarding the eligibility and appropriateness of client for hospice servicesConduct initial meeting with the patient and family over hospice service and philosophy and obtains consents for careProvide the initial comprehensive assessment of immediate physical, psychological, social and spiritual needs of the hospice patient and family and initiates appropriate interventionsDevelop an initial care plan that establishes immediate goals and interventions, based on nursing assessment and collaborates with the attending HMD and core teamMaintain regular communication with the intake coordinator, patient care managers, RN case manager and core team to review patients and help with managing needs (this may include unusual or potentially problematic patient/family issues)Timely submission of all documentationPerformance of other duties as requiredEnsure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and proceduresIt is this agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
