Hybrid Inpatient Case Manager

This is an opportunity to join a collaborative team environment with strong leadership support, a hybrid work model, and a focus on improving patient outcomes and care transitions.The Nurse Case Manager, RN (Inpatient) is responsible for coordinating high-quality, cost-effective, and medically appropriate care for hospitalized patients. This role focuses on concurrent review, utilization management, discharge planning, and care transitions to support safe and timely movement across levels of care.The Inpatient Case Manager collaborates closely with physicians, nurses, social workers, and interdisciplinary teams to optimize patient outcomes, reduce avoidable readmissions, and ensure appropriate utilization of healthcare resources.Key responsibilities:Performing admission and concurrent review using MCG guidelines to assess medical necessity and level of careCoordinating discharge planning and transitions to post-acute care settingsCollaborating with interdisciplinary teams to develop safe and effective care plansIdentifying barriers to discharge and implementing care coordination strategiesSupporting regulatory compliance, utilization management, and quality improvement initiativesFacilitating care conferences and interdisciplinary roundsEnsuring timely documentation and adherence to organizational and payer requirementsQualifications:Active NYS RN license requiredBSN preferred3–5+ years of acute care hospital experience requiredPrior experience in inpatient case management, utilization management, or care coordination strongly preferredStrong knowledge of MCG guidelines, discharge planning, and transitions of careExcellent communication, organizational, and critical thinking skillsCompensation: $95,000 – $105,000 base salary plus bonus potential
