Job Description Rehab Coordinator / COTA Job Description Position Title Rehab Coordinator / Certified Occupational Therapy Assistant (COTA) Department Rehabilitation Services Reports To Director of Rehabilitation (DOR) / Administrator Position Summary The Rehab Coordinator/COTA assists in the daily coordination and oversight of rehabilitation services within the skilled nursing facility while providing occupational therapy assistant services in accordance with physician orders, state practice acts, and facility policies. This role supports therapy operations, scheduling, interdisciplinary communication, compliance, and quality outcomes to ensure efficient and patient-centered rehabilitation services. Essential Duties and Responsibilities Clinical ResponsibilitiesProvide occupational therapy assistant treatment interventions under the supervision of a licensed Occupational Therapist. Implement treatment plans established by the evaluating therapist. Accurately document treatments, resident progress, and outcomes in a timely manner. Monitor resident response to treatment and communicate changes to supervising therapist and interdisciplinary team. Participate in care conferences, discharge planning, and family meetings as needed. Maintain compliance with Medicare, Medicaid, PDPM, and facility documentation standards. Coordination & Leadership ResponsibilitiesAssist with daily therapy scheduling and caseload management. Coordinate communication among therapy disciplines, nursing, social services, and administration. Monitor therapy delivery efficiency, productivity, and staffing needs. Support admissions screening and therapy evaluations for new residents. Assist with tracking Medicare Part A residents and rehabilitation utilization. Ensure timely completion of physician orders, recertifications, progress reports, and discharge documentation. Participate in quality assurance and performance improvement (QAPI) activities. Support compliance with federal, state, and facility regulations. Assist the DOR with staffing coordination, onboarding support, and department organization. Promote positive customer service and resident satisfaction. QualificationsCurrent California license as a Certified Occupational Therapy Assistant (COTA). Graduate of an accredited Occupational Therapy Assistant program. Minimum of 1–2 years SNF or post-acute rehabilitation experience preferred. Previous leadership or coordination experience preferred. Knowledge of Medicare Part A, PDPM, managed care, and skilled documentation standards preferred. Strong organizational, communication, and multitasking skills. Ability to work collaboratively within an interdisciplinary team. Physical RequirementsAbility to stand, walk, bend, lift, and transfer residents as required. Ability to lift up to 50 pounds with or without reasonable accommodation. Ability to perform repetitive movements and maintain activity throughout the workday. Work ScheduleFull-time salaried position Weekend or holiday coverage may be required based on facility needs. Preferred SkillsExperience with electronic medical records (EMR) systems. Knowledge of restorative nursing and rehabilitation programming. Experience in SNF operations and regulatory compliance. Leadership skills with ability to support staff and workflow management.
