From November 1- March 31, employees must provide proof of a flu vaccination before hire. Flu vaccines may be provided during new employee orientation at MBMHI. Requests for exemption to the flu vaccine must be submitted to Mary C. Young, CEO, for review prior to hire. All exemption requests are subject to CEO approval. Failure to comply with these requirements will constitute breach of contract, and the contract will be terminated. These documents will be required before NEO or provided by Infection Control by week 2 of NEO.
