The POE/ED CM is responsible for managing the patient flow through the ED through evaluation of incoming patients and assisting to direct the appropriate discharge disposition (Home, Post-acute Services, Inpatient or Observation) from the ED. This is done with an emphasis on early assessment of discharge needs as well as communication among the healthcare team members and the family/care givers.
