- The Wound, Ostomy, Continence (WOCN) Nurse is a clinical leader that is responsible for prevention, planning and providing care for patients with complicated wounds, ostomies, fistulas, presssure sores and incontinence nursing care at UCSF Benioff Children's Hospital San Francisco (BCHSF).
- The position works in collaboration with the Nursing Leadership, Faculty/providers, APNs and Nursing Staff at BCHSF. The WOCN will be responsible for the development of a comprehensive program for the prevention, care, education, and rehabilitation of children requiring wound, ostomy, and continence management.
- The WOCN will collaborate to formulate individualized treatment plans based on the needs of the patient and family, and oversee implementation of the treatment plan.
- The WOCN will refine or develop as necessary patient education tools, materials for APEX that will facilitate the education of patients/families in the management of wounds, ostomies, tubes, drains and wound vacs.
- The WOCN will document face-to-face encounters with clinical notes in the medical record.
