- The Home Options Path to Empowerment (H.O.P.E.) Program, managed through AgeOptions, is funded by the Department of Human Services under the Colbert and Williams Consent decrees is a nursing home transition program.
- As a Social Services Professional or Nurse in the H.O.P.E. Program, you will: Assess nursing home residents through interviews and evaluations with the resident, family and informal supports, and nursing facility staff; including reviewing medical records, to identify strengths, areas of development and independent living skills to support their transition to the community.
- Provide motivational interviewing to create harm reduction plans, crisis plans and 24-hour back up plans, as necessary.
