We are seeking a full-time, bilingual, and remote Licensed Vocational Nurse (LVN/LPN) to join our rapidly growing team as a Care Coordinator. You will be responsible for telephonically delivering high quality care, coaching, and appointment scheduling to assigned patients. Responsibilities: <ul><li> Remotely provide patient health coaching and care to improve patient outcomes</li><li> Coach and educate patients on improving their Chronic Conditions, preventive care and physician directives</li><li> Update and manage patient care plans based on assessment of patients’ needs and physician directives</li><li> Uphold scheduling flexibility between the hours of 8:00am – 7:00pm (PST) to call patients who live in different time zones </li></ul>Requirements: <ul><li> Valid Vocational Nurse (LVN/LPN) License</li><li> Valid Vocational Nurse (LVN/LPN) License - IN THE SATE OF NEW YORK IS MANDATORY (NY LICENSE)</li><li> 2 years+ Licensed Vocational Nurse (LVN/LPN) experience</li><li> Bilingual (Fluent in English and Spanish)</li><li> Experience working with EHRs/EMRs</li><li> Strong knowledge and background of chronic conditions</li><li> Ability to build rapport with patients</li><li> Compassionate and caring demeanor</li><li> Excellent documentation and verbal communication skills</li><li> Intermediate to advanced computer skills with the ability to navigate multiple systems</li><li> Reliable internet connection </li></ul>remote work
