- The Registered Nurse Director of Case Management oversees and provides comprehensive case management intervention for assigned caseload, including at a minimum quarterly documentation of the resident’s emotional, physical and psychological status, as well as assistance to residents and families in resolving issues and concerns as they occur.
- Complete required initial and ongoing evaluations and documentation on the Uniformed Assessment System (UAS) for assisted living program (ALP) residents.
- Work in conjunction with the Director of Clinical Services and LPN Nursing Supervisor to identify and continually evaluate each resident’s needs, interests, strengths and determine the capability of Valley Residential Services to meet those needs and then document this information on the Individual Service Plan (ISP).
- Communicate with the Home Health Aide Nursing Instructor/Supervisor regarding resident level of service changes and/or discharges.
- Assist the resident in obtaining and maintaining a primary care physician or source of medical care of the resident’s choice for the overall management of his/her health and mental health needs.
