Responsabilidades:
Position Description:The Home of Integrated Behavioral Health (HIBH) is the New York Foundling’s nationally recognized, award-winning community-based mental health clinic in East Harlem that responds to the needs of young people under stress with evidence-based mental health treatment affordable to a broad range of youth and families. The HIBH based Certified Psychiatric Nurse Practitioner is responsible for the provision of comprehensive, holistic, trauma informed, gender affirming, culturally humble and relevant, compassionate psychiatric care. Reports to the Medical Director of the Clinic, or covering leadership.Responsiblities<ul><li>Perform comprehensive psychiatric evaluations including review of referral information, engage participants, identify systemic strengths and weaknesses, and create individualized treatment plans.</li><li>Provide ongoing office-based psychotropic medication management as indicated to assigned caseload and provide coverage for peer's caseload as needed. </li><li>Identify and address safety/risk factors including but not limited to suicidal ideation, self-harming behaviors, homicidal ideation, and substance abuse. This includes the completion of safety plans and incident reports if necessary.</li><li>Engage and collaborate with all relevant key participants within each system to ensure their buy-in and alignment throughout treatment. </li><li>Provide direct clinical crisis intervention treatment using methods compatible with evidence based model principles and practices.</li><li>Engage with treatment team and care planning for outpatient mental health clients.</li><li>Participate in weekly interdisciplinary clinical planning conference and provide health education in staff setting.</li><li>Participate in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings.</li><li>Complete medical forms and reports legibly and in a timely manner for patients in assigned population.</li><li>Incorporate health education and illness prevention strategies in all encounters with patients and their parents.</li><li>Maintain clear, concise and timely documentation of treatment efforts in progress notes that demonstrate compliance with the assigned model and supervisory review feedback. </li><li>Review all rating scales data, health, school/education, and other relevant background documents submitted to the agency regarding assigned mental health clinic patient population.</li><li>Coordinate, collaborate and communicate with internal and external health and mental health, childcare and social service professionals responsible for the care of assigned population.</li><li>Keep abreast of the latest developments in psychiatric medicine through continuous review of the literature and through utilization of the time allotted for continuing education.</li><li>Identify community-based resources for families, collaborate and build relationships with stakeholders/providers (i.e., ACS, schools, doctors, mental health providers, courts, etc.) to monitor participation in services. </li><li>Provide On-Call coverage during Clinic After Hours.</li></ul>
