- Within the context of Patient Centered Medical Home (PCMH) policies and procedures and within standards of care for each respective specialty (Family Medicine, Internal Medicine, Pediatrics, Psychiatry, Obstetrics and Gynecology), the clinician will exercise individual initiative and independent action to ensure high quality and comprehensive primary care.
- The two central tenets of PCMH lie in team-based care and care coordination.
- The physician is expected to function as an integral part of the Primary Care Team (PCT), as a leader and an engaged team member.
