- Collaborates with pharmacy leadership, physicians and interdisciplinary team members in accordance with TJC, OSHA, Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations to deliver pharmaceutical care for eligible University Health System patients in the ambulatory setting.
- Conducts D.U.E. and patient/physician education activities as necessary.
- Serves as a primary preceptor for pharmacy students and residents.
- PharmD or Bachelor's of Science degree in Pharmacy required.
- Completion of an American Society of Health System Pharmacy PGY2 residency in ambulatory care preferred.
