- Role has two central tasks: palliative care case consultant (providing palliative expertise directly to patients and their families) and palliative clinician educator (helping RNs, APCs, physicians, and others develop primary palliative care skills).
- Care should be nuanced to disease trajectory and evolve based on disease progression; implement palliative care plans when appropriate
- Proactively partners and collaborates with TEC colleagues across multiple disciplines and settings (inpatient, skilled nursing, hospice, home health, specialty, etc) to create integrative, robust care plans.
- 20 - 30% Education & Facilitation to further palliative care best practices within the clinic setting
- With guidance from the Palliative Care Manager, provide informal and formal education for providers and staff around serious illness management, end of life care, clinical communication, effective goals of care conversations, interdisciplinary collaboration and other related topics
