- Reporting directly to the Project Manager, the full-time role of the Advanced Practice Nurse (APN) is to assess and evaluate patient primary and behavioral healthcare needs, and provide treatment onsite throughout the community at designated clinic site locations.
- Based on the clinical findings, the APN will provide medical treatment and schedule follow-up care or referral for specialty care, as needed.
- The APN will serve as the primary care provider for all patients admitted to service.
- To maintain round-the-clock continuity of care, the APN will assume a leadership role in the implementation of the telehealth technology that will be used to capture and store biometric clinical data, for transmission via mobile devices to the handheld, tablet or laptop devices of the Project Team clinicians.
- The APN will work in collaboration with faculty and preceptors in providing oversight for student clinical supervision.
