- Accountable for developing and achieving population health tactics and targets for the ACO, CIN, and other risk-based programs.
- In collaboration with clinical service lines, oversees transformation of clinical practice to achieve optimal outcomes and cost reduction for value work within the continuum of care (inpatient, outpatient, prevention, and wellness).
- Develops understanding of broad health landscape, determining key drivers and opportunities for innovative models targeting the Quadruple Aim. Uses qualitative and quantitative data to develop understanding of health needs and the attributes of the populations services to drive development of programs.
- Monitor and assess key performance metrics, including patient experience, outcomes, cost and operational efficiency.
- Acts as an advocate and provides ongoing communication with patients / families, staff and the multidisciplinary team regarding patient, staff, management, quality care issues and divisional / hospital goals.
