Requirements MINIMUM POSITION QUALIFICATIONS Associate Degree in Nursing. Unrestricted Registered Nurse Licensure in Washington State Excellent verbal and written communication. Excellent critical thinking and problem-solving skills. Moderate computer skills. Ability to work independently and as part of a team. Available to work on-call a minimum of two-holiday shifts per year. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions). Complete agency and departmental orientation and training program(s). Complete all continued learning, education, policy reviews, and required training by their respective deadlines. Maintain licensure(s) and certification(s) required for the position. RESPONSIBILITIES AND ESSENTIAL FUNCTIONS Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process. Provide skilled nursing assessment, planning, and care to maximize the comfort and health needs identified by the patient/family unit. Coordinates services to provide preventive and supportive care to the terminally ill and their families in accordance with approved care plans and Hospice policies and procedures. Directs the work of the Licensed Professional Nurse and Certified Nursing Assistant as assigned. Ensure compliance with IDG meetings at a minimum every 15 days and as needed. Patient care documentation entered into EMR within 24 hours. Patient care plan entered into EMR prior to Nursing Assistant assignment. Regularly attend scheduled staff meetings and in-services. Update facility staff on changes to care plans and maintain regular communication with administration staff at facilities of assigned patients. Utilize and order resources such as medical supplies, medications, and DME appropriately. Initiate communication with attending physician(s), other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family. Maintain regular communication with the Clinical Nursing Supervisor to review caseload. Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Seek input from other team members regarding the patient's plan of care to obtain additional knowledge and support. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated. Meet regularly with the interdisciplinary team members to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth. Attending patient care conferences and facilitating discussion of issues from caseload for full staff discussion, consultation, and evaluation. Supervise and document observance of the Certified Nursing Assistant's delivery of care every two weeks, and actual on-site observation every two months.</li><li>Inform the Clinical Nursing Supervisor of unusual, or potentially problematic, patient/family issues.</li><li>Assist other registered nurses including assuming primary responsibility for patients, when necessary.</li><li>Provide appropriate support at the time of death and perform bereavement assessment if requested.</li><li>Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.</li><li>Cross-train in other nursing areas as part of orientation and as needed/assigned.</li><li>Availability to cover after-hours on-call shifts as needed and as assigned.</li><li>Provide coverage or "float" to other nursing areas in the agency as needed and as assigned, particularly during the orientation period when patient caseload needs may be low.</li><li>Perform other duties as assigned.
