- Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital or med-surg clinical rotation in a nursing school or have at lease one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course approved by the Vice President of Home
- Assists with Independent Activities of daily living which may include light housekeeping, shopping, laundry and meal preparation, per the Plan of
- Accompany clients to appointments (transportation is clients responsibility unless driving waiver is signed and per the Plan of Care)
- Reports any and all changes in the clients condition to the Care Manager or
- Performs all activities as directed by the Care Manager or BOM
