- JOB SUMMARY: The Comprehensive Perinatal Health Worker (CPHW) is responsible for providing a wide range of culturally competent comprehensive perinatal services, promoting positive behavior change, and advocating for healthful pregnancy outcomes for qualifying pregnant women, from conception through 1 year postpartum, based on the State of California Medi-Cal Comprehensive Perinatal Services Program (CPSP) guidelines and protocols.
- The CPHW is part of an interdisciplinary team that includes the Medical Provider, Medical Assistant, Registered Nurse, Registered Dietitian Nutritionist, Social Worker, Health Educator, Pharmacist, Dentist, Care Coordination and Patient Services Representative.
