- The Collaborative Care Mental Health Clinician (CCMHC) is an integral member and provider of a patient's medical and mental health care team.
- The role serves as the cornerstone of the team consisting of a patient, the patient's primary care provider, and a psychiatric consultant.
- The CCMHC is responsible for providing triage, clinical assessment, direct diagnosis of mental health disorders in conjunction with DSM, ICD-10 and Collaborative Care and/or Psychotherapy codes, psychoeducation to patients and primary care providers, care coordination, case management, direct evidence-based psychotherapy, treatment plan formulation and updates, crisis coordination, and relapse prevention planning.
- The locations we are looking to cover are CVMC Green Mountain Family Practice (Northfield), CVMC Integrative Medicine (Montpelier), CVMC Family Medicine (Berlin) and CVMC Pediatric Primary Care (Berlin).
- Mental Health Counselor license, Social Worker license, or Marriage and Family Therapist license required.
