- The clinical pharmacist is responsible for working directly with patients, prescribers, other clinicians, payers, and pharmaceutical manufacturers to optimize medication therapy and patient health outcomes.
- The clinical pharmacist is also responsible for fulfilling the responsibilities of the staff pharmacist.
- The Clinical Pharmacist will potentially oversee the staff, clinical programs, policies, and procedures for the ambulatory infusion center (AIC) and home infusion therapy (HIT).
- The clinical pharmacist oversees the provision of the OptiMed patient management programs (PMP) and aims to improve patient-focused clinical outcomes using evidence-based resources, enhance patient understanding of their disease process and associated therapy, increase patient adherence to their medication regimen and treatment plan, and prevent medication misuse and detect and prevent adverse drug events.
- The clinical pharmacist performs comprehensive counseling with each patient assessing the patient for potential adherence issues, and educating the patient on appropriate medication use, storage/handling requirements, potential side effects, and any other requirements or pertinent information related to the patient’s therapy.
