- GENERAL SUMMARY: Provide a collaborative and inclusive approach to pastoral care by ministering holistically to the spiritual needs of patients, their families, medical staff and administration.
- Develop relationships with community faith leaders and cultural brokers, building the sources of support available for specific needsProvide institutional leadership through spiritual care education for all disciplines Support end-of-life decision making and provide spiritual support during the dying process.
- Perform Advanced Care Planning conversations and document in the EMR Provide in the moment bereavement support Collaborates with palliative social workers and nurses to perform bereavement follow-up calls and connect families with hospice and other community resources for follow-up bereavement support.
- Develop self-care activities to engage the palliative medicine team and other hospital staff in self-reflection regarding their own spirituality.
- Through structured activities and embedded in daily work, provides spiritual care education for the palliative care fellows, rotating learners, and team.
