Chaplain Hospice Home Health - Part-Time

- Home Care is defined as Home Health, Hospice, Home Palliative or any future at home service offerings as prescribed by KP. The Chaplain is a member of the interdisciplinary patient care team, participates in patient care planning and documents all pertinent interventions in the patient care record using established charting methodologies.
- May serve on Committees and work in collaboration with committees such as Bioethics, EAP, LCP, ELOA.Conducts In-services and CEU programs to improve Integrated Model of Care to include but not limited to New Employee Orientation and Interdisciplinary Meetings.
- Provides Staff support to maintain Spiritual Resilience in HealthCare to include but not limited to IDG & All Staff reflections, speakers and rituals.
- Provides faith/ value-based rites/rituals and maintains collaboration with Community Clergy to ensure support to staff, patients and families for religious specific spiritual care needs.
- Basic Qualifications:ExperienceMinimum of one (1) year of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Ambulatory, Home Health, Palliative Care or Hospice setting required.
