- Description Location: UCHealth UCHlth Harmony Campus 2121 Harmony, US:CO:Fort Collins Department: PVH Cancer Ctr Rockies FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks) Shift: Days Pay: $23.89 - $37.03 / hour.
- Responsibilities : Coordinates with home care agencies, post-acute care facilities, durable medical equipment companies, transportation agencies and others as indicated.
- Collaborates with the care management team for effective patient or client outreach, care coordination and hand-overs across the continuum of care.
- Completes referrals, provides educational resources and transitional care arrangement in support of Care Managers and Social Workers for patients recently discharged from UCHealth facilities.
- State licensure as a Practical Nurse (LPN) or Vocational Nurse (LVN).
