- Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has one of the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.
- General Purpose: This Licensed Master’s Level Clinician (LCSW, LPC, LMFT) provides clinical mental health services to patients in our highly integrated primary care setting.
- Deliver brief clinical interventions to STRIDE’s diverse patient population in an integrated primary care setting, including screening/assessment for behavioral health disorders, substance use concerns, stress, and/or behavioral challenges with medical regimen, and providing brief therapy services when indicated
- Contribute as a member of multidisciplinary teams (comprised of medical, dental, care coordination, health education and other BHPs) in delivering behavioral health services to STRIDE’s patients
- Colorado Licensed Clinical Social Worker (LCSW) in active and good standing; Colorado Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) in good standing will also be considered depending on previous experience in similar settings and site/location availability
