The Hospice division is experiencing significant growth, and we are looking for true leaders that are passionate about patients, families and providing care for the mind, body and spirit in a way that is comforting and compassionate.
Once a referral has been made to Heartland, the Liaison RN coordinates the care transition for patients requiring the services of any of the products offered in that market including hospice, home care, infusion and IPU services as applicable.
Heartland is a part of ProMedica Senior Care and is one of the largest not-for-profit senior living organization for skilled nursing, assisted living and hospice care.
You may be a candidate for this role if you are experienced in hospice, home health, oncology, chronic disease management, palliative care nursing, case management or discharge planning.
